State of Illinois Health Requirements for the 2026-2027 School Year

Students will NOT be able to attend school after October 15, 2026 until ALL required physical exams are received.

–Health requirements and exclusion criteria are determined by the state of Illinois. All public schools must comply and enforce these laws.(105 ILCS 5/27-8.1)

*Pre-K, Kindergarten, 6th Grade, and 9th Grade

Physical Exam (Sports physical Not acceptable, IL Certificate of Child Health Exam form is required) A signature with the date by the parent is needed upon turning it into the office Due October 15, 2026

Immunization Records will also need to be on file signed by a doctor, including (1) Tdap, (1) Meningococcal–(6th grade), (1) Meningococcal–(12th grade)- Due November 15, 2026.

*Kindergarten, 2nd Grade, 6th Grade 9th Grade

Dental Exam on IL form due May 1, 2027 (Can be dated within 18 months of May 1 deadline)-dental exams are not part of the October 15th deadline.

Kindergarten will also need vision exams from an optometrist October 15,2026

Any Religious or Medical Exemptions for Vaccinations with the Physician Signature are due by October 15, 2026

Any appointments scheduled after October 15, 2026 must be reported to the school office otherwise your child will be excluded beginning October 16, 2026.
